We currently offer supervisory care and personal care, with memory care being added soon. Supervisory care provides daily oversight and support for residents who are largely independent. Personal care includes hands-on help with daily activities like bathing, dressing, mobility, and medication management.

We’ve designed our admissions process to be as simple as possible for residents and families, while meeting all AZDHS requirements: 1) Tour & conversation — visit our community and share your loved one’s care needs. 2) Health documentation — a physician, nurse practitioner, or physician assistant completes a brief health assessment (dated within 90 days). We provide the form. 3) TB screening — a simple TB test or physician statement, completed within 7 days of move-in. 4) Residency agreement — we walk you through a clear, plain-language agreement before move-in. 5) Move-in — once paperwork and testing are complete, move-in can happen immediately. 6) Personalized service plan — within the first 14 days, we finalize an individualized care plan with you.

Palms Care Facility is largely all-inclusive. Our rate starts at $3,700/month and covers room, meals, and standard supervisory or personal care. The only additional fees are for higher-acuity needs, including memory care, heavy incontinence care, and continuous night monitoring.

We do not currently accept Medicaid or ALTCS. We ask families to plan for private-pay costs when considering Palms Care Facility.

Move-in can happen immediately once the required paperwork and health/TB testing are complete. There’s no lengthy waitlist process on our end — the timeline depends on how quickly a resident’s physician documentation can be gathered.
