Palms Care Facility partners with hospital discharge planners, social workers, geriatric care managers, and other healthcare professionals to help place clients in a supportive, licensed assisted living community in Casa Grande, AZ.

What We Offer

Supervisory and personal care, with memory care services coming soon. All-inclusive, predictable monthly pricing starting at $3,700/month, with add-ons limited to higher-acuity needs (memory care, heavy incontinence care, continuous night monitoring). Fast turnaround: move-in can happen immediately once required health documentation and TB screening are complete.

Admission Criteria

We accept residents who need supervisory or personal care support and do not require continuous medical or nursing services. Required documentation includes a physician/NP/PA-completed health assessment (dated within 90 days) and TB screening documentation (within 7 days of move-in).

Please Note

Palms Care Facility does not currently accept Medicaid or ALTCS. We are a private-pay community.
